TO QUALIFY
The patient must be under the care of a physician, podiatrist or dentist, be homebound and in need of intermittent skilled care. Home Health services consist of visits to the patient at home by professional and support personnel.
To receive this care the patient's physician must refer him or her to the agency by written or verbal orders. This may be done while the patient is in the hospital or at home. the doctor's order will request an evaluation for home care services.
If the patient qualifies, a plan of care will be developed by the physician and the home health staff, and then services will be provided accordingly. The Home Health Agency staff works closely with the patient's physician and communicates with him/her often about the patient's progress.
The number of visits made by the Agency is determined by the patient's need and the insurance carrier.

For patients who are covered by Medicare, Medicare will reimburse the agency for covered charges; an unlimited number of visits are approved for patients who are homebound. Private insurance companies vary the number of visits and amounts that they will cover.
The Home Health Staff will check with a patient's insurance company concerning coverage.
To be eligible for Home Health Services by Bright Horizons, a patient must live within a 30 mile radius of the agency.
