SKILLED NURSINGFull medical services of traditional home health for skilled observation and assessment and treatments such as wound care (wound vac), high-tech infusion, catheter care, diabetic management and many other nursing procedures. Services rendered by Registered Nurses and Licensed Vocational Nurses consistent with a treatment plan established by the patient's attending physician. Specialty Nurses trained in psychiatric treatment care and Enterstomal therapy are also available.

WOUND CAREThe Bright Horizons Home Health Services Wound Care Program is a patient centered wound program that takes a comprehensive approach to wound management and patient care. Our team of wound care trained and certified nurses work under the direction of the physician orders to provide the right care to get the best outcomes and healing potential for every wound. We are able to manage complex wounds, simple wounds, surgical wounds, diabetic wounds, vascular wounds, burns, ostomies, feeding tubes, and more.

CERTIFIED HOME HEALTH AIDESCHHA assist patients with activities of daily living and personal care. From grooming, bathing, preparing a light meal to light housekeeping, our staff is available for a vast number of personal care duties.

PHYSICAL THERAPYLicensed Physical Therapists and Physical Therapy Assistants work to promote a patient's physical function and improve range of motion. Therapists assist and/or train patients with gait training; use of wheelchairs, walkers, artificial limbs, etc., and pain control.

SPEECH THERAPYSpeech Language Pathologist conducts appropriate evaluations, assesses home environment (as appropriate), and identifies equipment needs relative to speech/language function. The Speech Language Pathologist identifies functional speech defects and establishes a plan of care to improve patient's function.

OCCUPATIONAL THERAPYLicensed Occupational Therapists and Occupational Therapist Assistants evaluate patients' functional status (muscle function, endurance, visual coordination, written and verbal communication skills, self care ability, work capacity, etc.). The Therapist evaluates the home environment for hazards or barriers to more independent living and identifies equipment needs. The Therapist develops a treatment program and establishes goals to improve function.

MEDICAL SOCIAL WORKERClinical Social Workers assess the psychosocial status of patients related to the patient's illness and environment. They provide information and referral services for patients and families/caregivers regarding environmental needs and community resources.

REGISTERED DIETICIANRegistered Dieticians develop and implement a nutritional plan based on an assessment of nutritional needs. They plan, organize, and conduct dietetic education for agency personnel. They function as member of the home care team with expertise in nutritional counseling.
